1. Passport size photograph.
2. signature.(on 3"*2" size Paper). (If the signature submitted does not tally with the signature given on the previous registration certificate, registration may be refused.)
3. Upload Degree/Provisional Certificate from university attested by any Gazetted Officer.
4. Upload C.L.C or Attempt Certificate attested by any Gazetted Officer.
5. Pay Online of Rs. 700/- (Rupees Seven Hundred.) per degree. (A delay fine for registration of PG degree @ Rs. 200/- per year per degree).
6. Upload Registration Certificate.
7. Upload Aadhaar Card.
8. Upload All Documents in One Pdf.
9. Maximum File Size is 5 MB.
10. The Permanent address mentioned as per your MBBS Registration Certificate has to be filled in the Application
If the address is to be change, then an affidavit has to be given as per the address of aadhaar card.
Note: After applying online you will have to come to the office with original documents for verification at the given time.
