8. Certificate of successful completion of M.C.I recognized compulsory rotating internship as per rules in different departments. Upload certificate attested by a Gazetted Officer.
9. 10+2 Marks sheet (Decision of marksheet verification fee will be decided after seeing your mark sheet).(for Science Subject with 50% marks)
10. Upload Provisional Registration Certificate.
11. Provisional Registration Certificate in Original Copy.
12. Upload all pages of Passport for verification of study period.
13. Pay Online of Rs. 20100/- (Twenty Thousand One Hundred.).
14. Any relaxation given during admission will need confirmation from MCI.
15. Upload Aadhaar Card.
16. Registration Certificate will be issued to the candidate personally with signature in receiving register.
17. Upload All Documents in One Pdf.
18. Maximum File Size is 5 MB.
19. The Permanent address mentioned as per your Provisional Registration Certificate has to be filled in the Application.
If the address is to be change, then an affidavit has to be given as per the address of aadhaar card.
Note: * After applying online you will have to come to the office with original documents for verification at the given time.
* Candidate has to upload origional scan copy of their Documents .
* Upload NOC from College/Hospital where Internship to be done .
