3. Upload Registration Certificate of primary qualification(MBBS).
4. Upload affidavit for change of Address in Registration.
5. Upload PAN Card.
6. Upload Aadhaar Card.
7. Upload 10+2 Marksheet.
8. Pay Online of Rs. 1000/-(Rupees One Thousand.)
9. Upload All Documents in One Pdf.
10. Maximum File Size is 5 MB.
Note: An affidavit has to be given as per the address of Aadhaar card for changing address in original Registration.
